Location: Dayton, Ohio

***Field-based RN Case Managers must be able to accommodate up to 80% daily field-based travel within the Dayton OH area***

Primary Responsibilities

• Visit members in Long term care setting
• Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
• Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
• Assist members with the transition from a care facility back to their home or other setting
• This role involves approximately 80% travel and you’ll need to be comfortable dealing with a full range of facility based environments.

M ost of the time you’ll work in collaboration with a Nurse Practitioner, so the ability to stay focused and motivated is essential.

Required Qualifications:

• Current, unrestricted RN Ohio State license
• 3+ years clinical experience in a hospital, acute care, long term care, hospice, direct care or case management
• Access to reliable transportation
• Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications

Preferred Qualifications:

• Case management experience including Certification in Case Management
• BSN
• Field based case management, case management in a facility based environment
• Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
• Bilingual skills
< li>Experience in Home & Community based or Long Term Care services delivery
